Patient identity verification

Enter the patient's identifiers to match their record across connected facilities. NHCIP resolves identity using the National Identification Number (NIN) and the NHCIP Patient ID.

1 Patient identifiers

At least one of NIN or NHCIP Patient ID is required. Providing both improves match confidence.

National Identification Number (NIN) — 11 digits

NIN must be exactly 11 digits.

NHCIP Patient ID — format NH-XXXXXXXX

Enter a valid NHCIP Patient ID (NH- followed by 8 characters).

2 Supporting demographics

Used to confirm the match and resolve duplicates. Must correspond to the identifiers above.
